Registered Nurse – Patient Aligned Care Team (Outpatient)

About the role

The Registered Nurse will join the Patient Aligned Care Team (PACT) at the Faleomavaega Eni Fa'aua'a Hunkin VA Clinic in Pago Pago, providing comprehensive outpatient care to veterans. The role focuses on health promotion, disease prevention, and chronic‑care management while ensuring high patient satisfaction.

Key responsibilities

• Manage assigned patient panels, utilizing registries to guarantee timely evaluation and access to care.
• Provide health education, coaching on wellness, disease prevention, and chronic‑care management.
• Collaborate in developing, implementing, and revising policies, procedures, and clinical guidelines.
• Evaluate outpatient outcomes of evidence‑based practices and report PACT program results at the service level.
• Support colleagues through knowledge sharing, constructive feedback, and fostering a safe, professional environment.

Required profile

• Graduate of an ACEN or CCNE accredited nursing program with a current, unrestricted RN license in a U.S. state, territory, or the District of Columbia.
• Demonstrated English language proficiency for written and spoken communication.
• Ability to triage and respond appropriately to emergencies.
• Commitment to patient‑centered care, self‑management education, and continuous professional development.
